I have the answer here without being a smart azz. Get the drops out of the frig and just place bottle on the floor next to you. Then, you invite your dog to lay down so you can scratch him or rub him, or whatever it is your puppy likes. For mine, a Great Dane, she liked to "turkey breast" which was laying on her back with her front legs sticking straight up and out. But, I digress...
You work to position her/him so that the ear is facing up. You may have to lift the flap, but, by then, the dog should be so relaxed from your treatment, that you just reach around and grab the drops and do it. I figured, it tickled, so I held the ear and kinda rubbed it. I kid you not, within a couple of doses, my gal used to actually lay down and present her ear to me. It was pretty hilarious to outsiders, I could command her to "present ear" when it was time for dosing, simply by showing her the bottle. Once they know it helps, it is quite easy. At first, ya just have to kinda sneek up on em. Good luck.

Ear infections are extremely painful but, you are responsible to insure the dog gets the needed treatment. I have muzzled my dogs during treatment. Any dog can be unpredictable when in pain. Don't just put drops in you have to clean the ear first. The drops will just build up and can increase the pain and infection. Always, clean, dry and then apply the medication as directed. Yes they will cry and maybe scream from the pain but not doing it properly will only hurt the dog for a longer period, can even cause the eardrum to burst.

We had this same problem. We have a Great Dane and he had an ear infection. My husband would hold his head (gently, not mean or hurting the dog, yet firm enough so he can't wiggle around too much) and then I would hold the ear flap and drop the drops in as quickly as possible. Then we would release his head, praise him, and go give him a treat. He didn't like it, but it got the job done. He also thinks that he's the alpha, but the gentle head lock technique works. Get someone to help you. Put on heavy gloves and hold him in a heavy towel. The other person puts the med in his ears. Talk gently and nicely to him. It won't do any good to talk to him like that, but it will make you feel better.
Just hold his head still and it won't take more than 1 minute. Hopefully you only have to do it two times a day.

If the meds are by mouth then try putting it in peanut butter.
As far as the drops in his ears you are just going to have to be the stronger of the two of you. Be firm but not mean, and hold his head between your legs and put the drops in his ears.

If you can't control your dog, then muzzle him. He needs his medication or he'll never get rid of the infection and the pain must be driving him crazy. If you don't have a muzzle, cut the leg off a pair of panty hose and use that. Tie it securely, but not tight enough to hurt him.

I have a Jack Russel Terrier and every time I try to do something he doen't like, he tries to bite me, so instead of getting mad or me getting hurt, I put on his muzzle and he can't bite me, I can cut his nails, clean his ears ect... with out any trouble. Take it off and give him a treat, and everyones happy!!!

Try getting your dog to relax first before giving him his meds. Talk softly and pet him gently while he is laying down. Then once his guard is down and he has become relaxing squirt it in his ear. Or if its chewable smoosh it into his treats. good luck

You need two people, one to hold him down, and another to give him the ear medicine.
It's true, you could also get a muzzle but then he might get even more aggresive.
