To file an accident report, go to your local police/sheriff's station and ask for the forms. Usually, accident reports must be filed with-in 10 days of the accident. Contact your local law enforcement office to see if you actually need to file a report, most states require a minimum dollar value worth of damage or a person injured befpre you must file a report. You will need the following information to file a report completely:

-Names, addresses, and license numbers (including state) of all drivers involved.
-Insurance information from each driver.
-Vehicle license plate and VIN # (usually found on the top of the dash or on the pink slip).
-The names and addresses of any person injured or killed in the accident.
-The name and address of any owner of property that was damaged (this includes vehicles if not owned by the driver opperating it).
-The date, time and location of the accident (use mile markers on highways or cross streets in urban areas).

The only other thing you will need is a memory of events as they happened in the accident. If you can't recall what happened exactly, be honest and say so. Remember to photocopy the form before sending it in. If it gets lost in the mail or if your insurance company needs a copy, it will save time to have a copy on hand.

To obtain a copy of a report already filed, contact the State Department of Motor Vehicles. You will need to know the date of the accident and at least one of the parties involved-if you can provide other useful information such as license plate numbers or VIN numbers from the vehicles, please do so. Accident reports are public information, but public does not mean free-expect to pay a fee to get the copy.

Go to the District where the accident occurred, fill out the forms they provide, pay a fee and wait for the mail. NYC is $15. Don't know Nassau's fee.

If the cops didn't come to the accident, you need to go report it. It won't mean jack in court b/c the cops didn't see the scene. If you did call the cops and they showed up, just go to the police station and request a copy. They will tell you what you need to get one.
