There isn't an crisp answer to this question. Why do people overeat to the point of morbid obesity, even though they know it has significant health risks? Why do people become alcoholics, even though it is absolutely proven to cause all sorts of damage to the liver -- not to mention make you act like an idiot?
The basic answer is because the behavior (smoking, eating, drinking) supplies short-term satisfaction. They make you feel good in the short term, and nearly everybody likes to feel good.
Most of us are pretty good at looking forwards at the long term consequences of a behavior and recognizing the advantages and disadvantages of it. Conversely, most of us are pretty bad about forsaking immediate gratification even when it contradicts what our own brain tells us is good (or bad) for us in the long run. The justification to ourselves of "well, yeah, I know that's bad/good for me, but I'll stop/start doing it tomorrow" is just too hard to resist for most people.

It's an addiction that is very difficult to quit and stay quit ( the impulse lasts forever).
But why are smokers targetted so often. Why do people eat fast food when they know the health risks. How about drinking alcohol, taking prescription drugs, non prescription drugs, not wearing a seatbelt. How about over worked people, people who are depressed or stressed. All these carry health risks why target people who smoke when so many others are walking time bombs .

Because it's fabulous.. Yeah, I know, smoking is bad for you, and so is drinking alcohol, eating greasy foods, driving without a seat belt, being a couch potato, shift work etc... So people just need to have a balance of everything. You can't be good all the time, you'd just end up regretting it all. When your health is jeopardized by your actions, then it's time to re-evaluate your choices..
