First you have to realize that you are human, and you were born into sin. No matter how much you try not to-and the key is to try and learn from your mistakes and not repeat them again-you will sin more often than not.
The best thing to do is forge you own relationship with God through prayer and talk to Him like you would your best friend because He should be one in your life as well. You dont have to get all formal with Him, that is the biggest misconception. Praying is just a conversation with God, nothing else. Also, read your bible for yourself. Really read and study the Word for yourself. Dont just go on what others say because they will steer your wrong more often than not (the devil was once an angel and does move about in the church house, too, wreaking havoc and chaos). Listen to your pastor and question. Ask plenty of questions until you understand and then read your bible again for clarity. Trust me this will make sense once you really get into it.
The best way to combat laziness...find something you want to do, create a clear and concise goal sheet mapping out every step to get it done, then do it. Time working on accomplishing your goals will kill the laziness. Also, make sure that your work is done to please the Lord. You can get away with slacking with your boss, but not God. He sees all. Give it your all, all of the time. God is your true CEO.
As far as your boyfriend...the issue of sex is something you two will have to combat together. Pray and talk about it together. If he is a Christian, then he definitely has guidelines he must follow as a man from the Bible just like a woman does. If you two are serious about your faith and your religion things will work out in God's time and you should really come to understand that.

Pray without ceasing.
It's a good sign that you are recognizing the fact that you're lapsing and that you want to change it.
Pray. Ask God to fill you with zeal again. Pray for God to give you and your boyfriend strength to remain pure. Actually, praying *with* your boyfriend about the purity issue would be a great idea.

Wow, I really affirm you for your desire to get back in step. I pray that God will encourage and assist you. First step is to get back into the Word, God's love letter to us. Next step is to get back into fellowship--church or Bible study or small group, whatever works for you. Listen to good Christian music. As far as getting passionate about God again...He wants us to be faithful even when the feelings aren't there, so keep on keeping on. BUT if you get a chance to go on a short-term mission trip somewhere, try to do it.That's the best way I know to really strengthen your faith, get closer to God, and see what He is doing around the world. I went through the same thing and I have found that we sometimes get so comfortable in a certain level of spirituality that we are afraid to go higher. I simply prayed and asked God to renew my spirit , my faith and to give me one heart and one vision. The answer is in prayer, studying your word, and fasting...
It is also good to get a prayer partner, my sister is mine.
I want to see how many Atheist are going to respect this question... or answer with sarcasm and hatred... since Atheist is are not Christians.

Lam 3 "The Lord is good to those whose hope is in him, to the one who seeks him; it is good to WAIT quietly for the salvtation of the Lord.. You could start reading the Bible, praying, and going to church. It will make you feel a lot better. Try getting things for others for Christmas this year too. It might seem weird, but you'll feel great seeing some one open that gift that you got for them and smiling. You'll feel proud of yourself. I bet your brothers and sisters would feel great to have their sis back.

Well your already closer to God then you think. You have admitted your problem and your need for Him. He is waiting for you just to come to Him and surrender all. Lay all your problems down for Him. He will come and show you His love. As you encounter more and more of Him you will be made into more and more of His image. As you experiance more of Him, the hunger for more of Him will grow. Start with this; the old testament is the law of Moses; not Genesis-Malachi. (Can one have the law of Moses before Moses was born)?
How many things in Genesis was done away? (Like the Sun, moon, and stars, animals and plants (Gen 1); the pain of child birth (ch. 3), the rainbow (ch.9), many languages (ch. 9), etc?
The old testament was done away (Hebrews).
