Why would a simple question be considered "gay"? I'm not too sure you even know what the word means. "How are you?" is a perfectly normal expression used in everyday speech.

A lot of people say this but the problem is that they do not listen to the answer.
Hello or Hi, How are you. It shows concern about their welfare without being intrusive, but do listen to the answer.
