I highly recommend Sonico Red card. Within USA the rate with local access and maintenance fee is 1 cent/min. No connection fee. Rechargeable. Pinless dialing.

The best rate is 1 cent/min with maintenance fee (cheap call card) or 1.5 cents/min without maintenance fee (Jupiter card).
