Whow!! I can't believe some of the answers. With what goes through some peoples mind I wonder how they could respect themselves.
A gift certificate is nice perhaps for a massage :) Also, a workout outfit based on the colors and style he usually wears. A gym bag with a few towels, a 6 pack of his favorite water, $10 gift card to your local nutrition store, a t-shirt & a card. You don't want to spend to much set a limit & stick to it, maybe $30-40 or more depending on how excellent he is at his training profession :)

I train a lot of people, and for I would love for someone to get me a Christmas gift...period.
Try giving him either a new tank top or maybe some new wind pants, sometimes we go through 2-3 sets of clothes in a day.

If I was a personal trainer, and you were fine ,I would say a lap dance should work out just fine, and then some happy hour at a local eater,to get to know you just a little better:)
