Once is more than enough. It doesn't mean that having sex one time will always cause a pregnancy, but one time, it is a roll of the dice. You run a better than 50/50 chance of a pregnancy.
Even with a condom, you look at about a 10-15% chance of failure, causing a pregnancy.
