on your thigh closes to the server , you then open the napkin
in your lap ..... you are a woman that napkin stays there.

when your food arrives you request a additional napkin.

You can do it either way. However, if I order a drink that's not clear (such as red wine), I usually put the napkin on my lap as soon as my drink arrives. Accidents do happen, so it's just a good idea. Also, if the waiter brings a bread basket and I plan to have a piece of bread, I put the napkin on my lap to catch the crumbs, etc. Otherwise, you can just put it on your lap when your food arrives. If the dinner conversation is good, then you probably won't even realize whether or not you've placed it on your lap, so if you've neglected to do so already consider that a good thing.

"Q: What do I do with my napkin?
A: As soon as everyone is seated, unfold your napkin and place it across your lap, folded, with the fold toward you. Do this discreetly without flourish. If you need to leave the table, place your napkin on your chair, folded loosely (NEVER wadded). Only after the meal is over should you place your napkin on the table to the left side of your plate (NEVER on your plate!)."

I don't follow these rules but apparently this is the proper way to do it. I don't touch the napkin until the food arrives.

After you order, before the food arrives. Follow your hosts lead/or watch the other guests. If you are going to be wrong, at least be consistant.

You should put your napkin on your lap when you're seated.

In an up-scale restaurant if you don't do it your server will.

Once you sit down you may put it on. In some restaurants, the waiter will do it for you once you sit down.
