A pre exhisting clause pretty much states that anything you've been treated for by a medical provider prior to your insurance coverage, will not be covered. This can be a temporary or permanate thing. Many policies have a no preexhisting clause for the first year of coverage. Keep in mind that it's only a pre-exhisting condition if you have been treated for that condition by a medical provider.

if you knew of your pre-exsiting condition, the insurance company will give you nothing, but if you can prove you knew nothing about it then they have to pay, this may have to be backed up by your doctor stating that you have never been treated for the condition.
