In the part where we are supposed to enter our awards, I don't understand why the columns say ACTIVITY, GRADE, AWARD. I'm just not sure what they want.

Yep I applied. You would say like "Soccer" then "9, 10, 11", then "All-Conference 2nd Team".

Take a step back and think.

1. People are reading this all over the world.

2. There are many educational (and other) institutions which have "Notre Dame" in their name

3. All we can know is that you're applying to what is presumably a Roman Catholic institution of some sort, probably educational.

4. It's possible there's more than one application form for different courses at the same institution

5. Put yourself in the position of someone who doesn't know you, where you come from or where you're wanting to go to. Then ask the question and you may get some meaningful help.

6. Phone up whatever institution it may be and ask for the admissions department and get some clarification from them as to what's being looked for in that section of the form.

Activity: Spelling Bee
Grade: 9th (when you did it)
Award: first in state..

at least that's how I'd do it
