You can go to your dmv website. A lot of states have a form you can just fill out online.

Each state has their own rules and regulations about personalized plates. And each state has their own website to visit to find out how to get them. States charge anywhere from 25.00 to 100.00 for standard personalized plates. States that have different "cause" plates, IE Cancer, Organ Donation, Military, Wildlife, etc, will also charge the normal fee for those plates each year. Most states the personalization fee is only for the initial plate. Have a number of different plates in mind because so many are already taken. The application usually asks for first preference, second preference, third preference etc.

The DMV has a form you can fill out for it. Like in CA, we have about 20 different styles of plates, and then you can get a personalized one on top of that. It's only like a hundred bucks - that's why everyone does it!

Go to the DMV. You will have to pay extra money and you can't use plates that someone else has already (obviously), so you may not get your first choice.

Go to the DMV and ask to use their computer for the available plate numbers, then u ask to change it and i think they charge you $50, at least thats how you do it in california.

go to the DMV, they will show you a list of plates (like to help children and stuff) to choose from, and then you must have about 10 different combinations of letters that you are willing to accept.

There is a form that you have to fill out and have approved to ensure you are not asking to put something inappropriate on your plates. Just go wherever it is that you go in your state to renew your tags and request a form.
