Generally no. Beer is made of four things. Hops, malted grain, water and yeast. These contain no caffeine. However, Anheiser-Busch has started making a hybrid beer/energy drink. That does contains caffeine. It's the only one that I know of.

No, but several of the large distributors have begun marketing a beer with caffeine, among other things, added as sort of an energy drink beer.
Budweiser's is called Bud-E.

Beer does not, by default, have caffeine. Cola has caffeine because it is naturally found in the Cocao plant that it comes from. Beer is made from grains that do not contain caffiene.

Technically no. You only need four ingredients to make beer, malted barley, hops, yeast and water.
However I have heard some major beer distributors add caffeine and since the list of ingredients is not on the can...well you know.

No, and adding caffeine will disqualify it from being considered a "beer." It will have to be called a "malt beverage." Sparks and Tilt are two brands that do this.
