2) The best way to ask someone out is to simply be direct.
3) Fourteen year olds aren't usually well schooled in how to politely turn someone down. If she's one of them, don't take it personally...there's a jillion girls out there and she's only one of them.

Get to know her a little, then ask her out. All she can say is yes or no. It's that simple. Everybody gets rejected so don't let it hurt you if she says no. Just say ok, maybe some other time then smile and walk off. Continue to talk to her and maybe she will start sending you signals that she wants you to ask her out again. Just be natural about it.

Tell her this..
"Hey cutie, I happen to know someone who might like you, that is, if you're more than just a pretty face. He's funny, charming, and I think you might really like him. *Wink* I could give him your phone number if you want to write it down. I'll make sure he gives you a call later and that they two of you get together for lunch or something"
Make sure you have a pen and paper handy so she can write her info down. If you don't want to ask for a number, maybe an e-mail.

You could start of with something small like a small teddy bear and approach her calmly give her the gift and say Will you go out with me (if she says no don't take it to emotionally-its happened to me before)

Start off with a convorsation. If you get along, casually ask her out. If it doesn't go as well, start the convorsation again next time you run into her.

well you gotta always be nice to her and don't be afraid to talk to her. after a couple of weeks then you just ask her out. you do that by saying if she wants to go get something to eat or if she wants to go watch a movie. (if she wants to watch a movie, and she didn't already see it, then take her to see happy feet, girls love that movie.)
