For Pete's sake, if you cannot be honest with your spouse, then you shouldn't be married. Just say so in a tactful manner

I assume you mean type 2 and not type 1. I think the only way is the the direct way. Schedule some time, turn off the phone, empty your bladder and sit her down. "Honey, I have to tell you something..." Is this a condition you recently aquired? If so, you will have to explain that and that may become the new focus of the discussion.
