How do tv stations/shows know how many people tuned in to determine ratings

Nielson ratings.

These operate two-fold:

1. The Nielson people install devices on a certain number of TV sets and record how much and what is viewed on those particular TVs. (Or at least what was playing on them, whether they were actually being viewed or not.)

2. This I know from personal experience; I spent one week once as a "Nielson family." They send you a booklet for each TV in your home and you write down every thing you watch in great detail.
Every hour is recorded by you. The hours you aren't watching you draw a line through, and write down the details for the times and programming while your TV is on.

Neilson ratings are conducted by means of electronic boxes attached to television sets of some TVs with the approval of the household. Other families are asked to keep diaries of what the family watches. These households are carefully chosen to represent all of America.
