Call the courthouse and ask them. If you approach them first they'll be more forgiving than if they have to call you.

You should call the jury services office and tell them you forgot. They will probably set another jury date for you. They won't come after you, fine you, or jail you unless it becomes apparent that you have no intention of cooperating and appearing as required. They know human beings are not perfect and will give you another chance if you merely forgot.
Eventually, if you kept on "forgetting", over and over again, a judge could issue a bench warrant and you could be arrested and brought before a judge to explain your failure to appear. The judge COULD issue a fine or even jail you. But he will not do that unless you make it very obvious to him that you are intentionally being uncooperative.
If you just forgot, don't worry about it. Just call and tell them.
