The most romantic and memorable option is the last on list,thats surprise her by telling her to shave your head on new year eve.
I would go head over heels if asked by my bf to shave his head on a very special day and make it last in memory for life time.She will never ever forget it.
But i have to tell you that all other ideas are cool either.

LOL.. I don't think she wud Like that Present.. MayB U shud take her with U when u go for shaving ur Head.. MayB she wud like that more, rather then U suprising her.. Atleast She wud Feel that U trust her even in all BAD times.. Like shavin ur Head..
