The previous person was mainly talking about other areas near Macau but not in it. I say, stay in the city if you're just going to be there for a day.
Start with the ruins of Saint Paul's Cathedral and Largo de Senado. That's the main tourist hub and certainly worth a visit. Explore some of the side streets in that area as well; there are some cool little cafes and great shopping.
After that, maybe head over to the Wine Museum and/or Grand Prix Museum. The two share the same building and you get a discount if you go to both. I'm not into racing, so I just went to the Wine one, but it's quite nice and they let you sample some of the ports.
After that, just get out and explore. Pop into a casino or two, try some of the local restaurants, that kind of stuff.

Macau is a unique city with the charm of the Chinese and Portuguese heritages! You can visit the The Ruins of St. Paul's, which was a beautiful Roman Catholic Church built in 1602, and the Museum of Macau where you have a idea about history and bi-culture of the city. Of course, if you like you can always try your luck in the world class casinos.
Second option is to visit Guangzhou, which is close by and has much more historic and cultural value. You can visit Dr. Sun Yat-Sen's Memorial Hall, who is "the father of modern China"! Then visit Temple of the Six Banyan Trees and Chen Clan Academy. These are some of the more famous sights in the city that will give you an idea of the glorious history in China!
Third option is Hong Kong which is only an hour by Jetcat (boat) : you can visit Victoria Peak (by Tram) The bird's eye view of the harbor and the Kowloon Peninsula from this 373 meters high vantage point is really breath-taking! You can also visit Aberdeen Fishing Village, where you can get a glimpse of the life style of Hong Kong fishing folks. Afterwards, you can stroll in Stanley Market to get some good bargains and nice souvenirs.

Macau is actually very small and if you don't get hung up in any one place too long, you can do a whirlwind tour of the entire city in 24 hours. That is unless you got trapped inside one of the casinos. See the following link.
