I can only comment on Eurpean clubs as I do not know South American clubs.
At the moment it is between Milan and Barcelona.
Overall is is close between R Madrid and Juventus with my vote to Juventus :-)

Presently I think FC Internazionale (Inter Milan).
All-time: Real Madrid although they are playing poorly right now.
Barcelona is still in the hunt.
However Internacional of Brazil has won the World Club Cup. So on paper I think its Internacional.
But for me personally, the hottest club right now is Inter-Milan.
