What does the mom you're buying for like to do? Is it your mom, or the mom of a significant other? Does she collect anything or have a favorite hobby?
The gifts that are always the biggest hit with my mother are the ones I put creative effort into; nothing expensive. I actually love to make gift baskets full of inexpensive and homemade items for her, and they work for both Christmas and her birthday. Best of all, you don't really have to wrap anything, so hardly any clean-up.
Does she enjoy gardening or flowers? It may be a bit hard to do in the winter, but it can be done. Start with a wicker laundry basket, and then hit up your local hardware store or greenhouse (even the gardening center at Wal-Mart would work). Pick up a small trowel, some gardening gloves, and a few packets of flower seeds. If there are any available in your area this time of year (depending on where you live), you can even pick up a few small, live flowers. Add a small watering can for a nice touch, and arrange everything in the basket (you can even line the bottom of the basket with fake dried grass and such from a craft store). Slap a bow on the basket and include a handwritten card, and you're good to go. I did this for my mom's birthday last April, and she absolutely loved it.
Instead of spending money for a spa gift certificate, you can always give her a home spa. Fill a basket with a book by her favorite author (or a gift certificate to a book store so that she can choose her own), along with sweet-smelling bubble bath, lotion, candles, and maybe a bathrobe and/or a pair of fuzzy slippers (these are so inexpensive now, it's crazy). Include a hand-printed coupon for a free day at the "spa."
A movie buff? Buy her a DVD and include a box of popcorn, some favorite movie candy, and a small gift certificate to her favorite pizza place.
A letter-writer? Try some nice stationary, along with a pretty pen set. Maybe a small booklet of matching stamps and some nice stickers.
You can even look around the house and see if you can find anything your mom needs but hasn't mentioned. For example, this year I found a small wicker basket (actually a trash can, haha), and I'm filling it with some nice dish towels and a few new kitchen tools that my mom desperately needs (hers are old and worn). I picked up some new wooden spoons, a couple of spatulas, and a decent pair of kitchen scissors for her, and total (including the basket and shred to line it), I spent less than $20. The bonus is that nothing will go to waste; my mother is a quilter, and I'm sure that the basket will be used to help hold her quilting scraps.
It doesn't have to be expensive, or even something that costs any money (an added note: never underestimate the power of your local dollar store). It just takes a little creativity. I promise you, the mother you're trying to find a gift for will appreciate anything you give her, even if it's just a card with the promise to do the dishes every night for a month. It's the little things that matter most.

If it's a mom you know very well, get her a gift card (or two or three) for her favorite store(s). Or, for a more personal touch, make her something she needs. Maybe an oven mitt or magnets for the fridge. get creative! And make sure to have fun! : )

*A homemade gift such as a nice picture of you and perhaps a short note or poem expressing what she means to you.
*One of her favorite books, fuzzy socks, candles, bubble bath.
*A coupon with several dates for her to pick from where you take her out for lunch or dinner.
Moms give us our best gift---life....though difficult to come up with that special gift for Mom, the effort is worth it. Using your own creative talent and/or imagination, and she will love anything you come up with because it was done with love and care.
