First of all, if at all possible, get off the internet *unless you need it for your book report.* Being online is distracting for a lot of people (because you can always navigate away from what you're doing).
Just breathe for a second. You still have time to do this. At least you know that you are procrastinating, and you can still head this off.
Now do it. Just write your first sentence, stop thinking about it too much. Go from there. Write a first draft, no matter how bad it is, and then you will have something to go off and maybe that will help you focus.
If you can't focus, maybe you should burn off some excess energy by doing some exercises or just sitting quietly for a few minutes.
Once you actually get going it won't seem so bad.

If you don't need your computer for the report, TURN IT OFF AND HIDE IT UNDER YOUR BED. Or something. And if you don't need it for research, you don't need it for your report, just write the rough draft by hand. And you really don't need it for research anyway. Get a book.
After that, make yourself a schedule. Say that you'll work on your report for 45 minutes without stopping. Then you'll get a 15 minute break to do whatever you want. Then work for another 30 minutes, then take an hour break. I mean, you don't have to put too much concentrated time into it since you still have time. But definitely work for a an hour or something every day.

Break it down into little steps. You say that you know what to write. Keep building on those initial sentences. How long does the book report have to be? You still have more than 10 days before school begins again. If you write a little a day, you'll be done in no time.

i think you should just get to it, first of all get off the computer! then get a calander and figure out what you should do on each day (example: mon: outline, tues: RD #1, etc.)
