This is a history question, not a government question. The answer you are looking for is D). The Columbia explosion was NOT caused by a malfunction. The tiles that dissipate heat on reentry were damaged by falling debris during takeoff. This resulted in overheating upon return. Calling this a malfunction is like saying your car malfunctioned when you ran over a spike on the road.
