When someone ends a sentence, or responds to a comment made by another person with "and how!", they are accentuating the truth of the preceding statement, ie:
"That was a big baby"
"And how!"
This indicates the baby was enormous.
-or-
"I sure would like to meet that good looking person."
"And how!"
This indicates that although the first person would like to meet the good looking person, the respondent's desire to meet them is equal or greater.

It seems that she may have been dictating something to be typed and in that context the use of "init" would indicate the end of a sentence. This would be an aid to the typist.
