Mankind can be a stupid entity. People who drink and drive think they are in control though probably not or just are not concerned about hurting someone else or getting "caught."
Then when an accident happens, they are very sorry. Why be sorry in the first place. If you drink, don't drive. Have a designated driver or use a cab (cheaper than getting a lawyer to represent you and paying higher insurance preminums) or check and see if there is a local organization that you can pay to have them take you and your car home safely.
You asked if they know what they are doing. Yes, they know they are driving after drinking and playing Russian roulette with their car, their lives, and the lives and property of others.

Because they are foolish idiots! I think that is so stupid. People know the risks of drinking and driving and I can not believe they are stupid enough not to care, or so stupid enough to think that it can't happen to them.
Less than two months ago my stupid uncle got into a car crash and killed a man all because he decided to go drinking and then driving. I've never known my uncle all that well, but I am so ashamed of him. How could he be such a stupid idiot? I hear from my family all the time how the wife of that man calls my uncle every day on the phone and reminds him on a daily basis that she no longer has a husband and her kids no longer have a father because of him. He feels bad about it now but how dare he! No words can describe how terrible I feel and how sorry I am for that family. I am so ashamed of my uncle and I can't believe that I have such a stupid person in my own family. That I can never understand how people are so stupid and careless as to not face reality and realize the dangers that drinking and driving can cause.

A simple question but a very complex answer.
The reasons vary. Some people can't stop drinking. There was one responder that said they can go to rehab. But rehab is very expensive and quite scarce for the number of people that need rehab.
Alcoholism is a disease that has more to do with the thinking than with the drinking. First they must put down the bottle and that is hard for an alcoholic. Yes, it is a crutch, but to the alcoholic it is as natural as breathing.
Not that we should make excuses for their behavior, (and mine, I'm in recovery over 7 years), they need to take responsibility for the disease although they didn't cause it, they are responsible for managing it.

They drink and drive because some people have a lot of problems and deal with it by using a crutch for them to fall on because it's there stupidity to drink and drive and they can go to rehab but are just too drunk to even go or they just don't want to people drink and drive to make themselves block out the world around them and do what they want to.

They think they can handle it. As long as they don't pass out and can stand on their feet, they'll be OK. Some people just can't abide someone else driving their car. They might wreck it.

I could be wrong, but I think fewer people drink and drive nowadays since the laws really crack down on it.

Because they are idiots. Unfortunatley is usally takes a horrible accident or death for some people to realize that it's wrong.

They are not in the right mind, they do not think when they drink, and sometimes it takes an accident to get them to think right, and sometimes that does not help.

Because they are just"plaine foolish" and some what to see just what they can" get away" with. Do not do it and do not ride with someone that is.
