Unfortunately government, you have people who under the guise of helping the people form bonds with lobbyists and take kickbacks or outright use their position to steal from the people and gain money and power for themselves. You read about a corrupt politician almost every day doing something.
Heck I have even heard of police officers selling drugs, and after the changes occur in congress and the house you will see ousted politicians going to work for powerful companies who they helped while in office for huge sums of money. So my answer is government officials or the government, and not just the American government.

The Italians are the most famous. To be honest it think it would be a toss up between the Russian Mob and the Japanese Yakuza. Followed by the Italian La Costa Nostra. Then you have your Tong, Triad, and Irish Mob. I think the most ruthless would be the Russians.

The Japanese Yakuza. They are world famous, yet untouchable. Even children know about them. In fact, cartoon characters have to have 4 fingers in all Japanese cartoons because it reminds children of the punishment the Yakuza give people who betray them.

The Sicilian Mafia. They patented the term "mafia." You hear about Russian Mafia, Japanese Mafia, Albanian Mafia etc etc, but really, there is only one Mafia that all the other mafias try to copy. So I would say that makes the Mafia the most powerful. At least linguistically. :)
