has it ever been suspended or revoked? ok not ok

how should i answer third question if my license is clean?

The wording is a bit confusing. Perhaps you should ask the person giving the interview to clarify that particular question. Those questions should be "yes" or "no," not "ok not ok." Maybe you should write "My license is ok" by it. Good luck!

Cross out the "ok not ok" and write down no. If they are confused by why you did this, or have a problem with correctly answering the question, then I suggest not working for them. If you don't feel like doing that, try circleing the "not ok" part.

It seems that they should check their questionares.
