Doctors can usually buy whatever they want, the chocolates are nice, but a concise, sincere, meaningful card will be appreciated and treasured. Normally I wouldn't put God in there not knowing the doctor's feelings on the subject but since you put that he mentioned God's influence on his work, how about:

God was the Driver, but you were the mechanic. Thanks for helping me take the (best/wildest/most important) ride of my life - motherhood!

God has a lot to do - so he created fertility specialists to help do His work. Thanks for helping God (and me!) conceive this baby.

These are just two that came to mind.

Thank you for all of the care and consideration you have given me and my family during this exciting time in our lives. All of the hard work you have done to help us conceive/try to conceive means so much to me/us, and we really appreciate you because of it. You have helped make one of greatest dreams of having a child come true, and I/we can never thank you enough. (Then some kind of closing like Hope your holidays are great or Best wishes to you and your family, etc).

Dear Doctor,
Please accept these chocolates as a small token of my immense appreciation and gratitude of helping my husband and i conceive. You've given us the biggest and best gift ever- a child, for which there aren't words to express our sincere gratitude. God bless you!
Sincerely,
