They are extremely difficult to manage with any type of rhythm even if you are in shape. The most difficult push up I have seen was where a dude did a one-arm push up from a hand stand. Try that some time.

It depends on our level of strength
If your tricept is really strong and you can support the majority of your body weight with the outside of one arm; then no they are not hard
but for most of us the are very hard

it takes practice. I don't really do them as there are much better exercises though.
