I've had that happened to me a couple of times one where i was in Manhattan and this random guy came up to me and was like I have to show you something. I was in the middle of the sidewalk with a friend so I wasn't nervous or anything I was curious if he was really going to do what I thought so I looked and he put his hand down his pants and pulled out a 40oz of beer. I just laughed at him. The other one the guy was drunk and was being all retarted and pulled out his dick to show me how big he was. I said ok I see put it away now. If I was with people and didn't feel threatened I would be like whatever but if I was alone or felt uncomfortable I would walk away very quickly.

Context is very important.
If I were a urologist, I'd find it frustrating if a patient refused to let me examine him.
If I were the significant (gender non-specific) partner, I'd take it as an invitation to get intimate and consider other factors such as environment (in a shopping mall?) and mood.
But if it was some dude I don't know, then a verbal threat for him to desist would first be issued. If he persists, I'd move to a place with lots of people and then call attention to him.
Under no circumstances should you attempt to show your indignation by kicking him in the gonads.
