You can get an executive Plus Mastercard at this link that I have found. All you have to do is fill out the survey and you will recieve the card. I applied myself on that site and all I had to do was fill out my information, it was quite easy and I should be getting the card soon. What this 'other' means is not specified in this text.

You can use PayPal (even though they`re thieves so WATCH OUT!), different credit cards, etc.
