Also how would you do this without seeming like you were just reading questions that you had just got off the Internet.

you don't read questions you just got off of the internet-- you ask about her.
talk about her
how is she?
what's she up to?
how has your [period of time] been?
and then follow up--- like keep talking about her--- untill she starts talking about you then talk about you... that's called a conversation.
don't try to script it.

the first conversation will always be a little weird....just make sure you don't let it go too long before a second call. The calls will get easier as you go....just be yourself. You might even just break the ice by stating the obvious....that a first call, (like a first date) can be a little awkward.... wish there was some magical list of questions to tell you, but just be yourself ... I'm sure you'll be fine.

Just talk to her like you would any of your friends. You don't need to read questions to have a good conversation! Just breathe...and be yourself
