Go to your local bike store. They will size you properly on the bike and show you the different options for bikes depending on what kind of riding you are looking to do. Do not go to your big box stores like Walmart, Target and such, they have the belief that one size fits all. All adult bikes for men are one size and women another.

One response talks about 20", 24" and 26" bikes. They are inccorect in their assumptions though, the sizes refer to tire sizes and are used to describe bikes for kids 20" tires, 24" tires. When you get into adult frames which have either a 26", 27", 700c or 650c wheel you start talking frame sizes. Frames will vary from as small as a 12.5" frame up to a 23.5" frame or a little bigger depending on the manufacturers.

As you can see there are a number of variances with the tire sizes and sizes of frames. It is hard without seeing a person on a bike and knowing what type of riding they are looking to do for someone to accurately size someone on a bike over the internet.

Your local bike shop can help with all of these issues.

You should be able to straddle the seat comfortably and when peddling you should not have to hyper-extend your legs/lock your knees.

Ideally, you should try out a bicycle and consult with an experienced sales person at an established shop.

Read the Steering & Seating section of the Wikipedia entry for bicycles. (Link provided below.)

It depends on your height (or the height of the person you're buying for). You really need to try them out at the place you're buying from. If it's from an actual bicycle shop, they should be very knowledgable about what "fits" you.

It depends on what you want to do with it.
If you want to do dirt racing, a 20 inch, same if you plan on doing skate park riding. if you just riding from point a to point b get a 26 inch if your over 5.2 feet and a 24 inch if your under. If your rteally crazzy and want tu jump off roves n stuff or do down hill trails get a hard tail - just ask your local bike store about them.
