It really depends a lot on your age.
If you are less than 16 years old, what would work really well for you would dbe to watch a lot of American tv, read the same book (in English, of course), more than once, and talk to people who speak English fluently. Also, going to school and interacting with kids that speak fluent English, and having a friend that speaks the same language you do is very helpful, because they can translate things for you, and you can ask them how to say certain things. This way you learn new words. Eventually, you will start to get used to English and speak it fluently, and you might even lose whatever accent you have. It's harder to lose the accent, but the most important thing is that you speak the language fluently. This is the method that worked for me.
Now, if you are older than the age I mentioned above (16), you might want to look for a tutor or take an English class. The nearest community college, or even high school, probably offers unexpensive English language courses. If you're native language is Spanish, a good option would be to buy the highly advertised video course, Ingles sin Barreras. You should also talk frequently to people who speak English fluently so you can practice your skills.
Like I said before, the method you would learn best depends on your age. Now it is up to you to choose what you will do to improve your English.

Buy a voice recorder and keep speaking english to yourself. Practice reading newspaper out loud.
Then watch more english tv. And then practice your english with some english speaking friends

Practice listening and speaking with native speakers. Also, listen to English radio, television, and movies. Practice is the only way. And in person practice is the best.

To completely immerse yourself in the culture and media of the English language. That doesn't just work for English, but any language. I would listen to Spanish music, watch t.v., practice with native speakers and find myself thinking in that language sometimes.

Most definitely carry an English dictionary with you- try to learn a word a day and use it correctly in a sentence. That's what I did when I was learning Spanish!

Have conversations often with english speakers, read alot more and its "fluently"... but thats ok I've seen english speakers make bigger mistakes.
