the problem with this question is, your friend wouldn't say that to you. How do you do? is NOT the same as how are you? Rather, it is said in formal introductions and doesn't require an answer. Saying it is polite and kinda like a pleased to meet you. You DON'T reply back with how you are feeling.

If he is a friend he would know you.Therfore he would never say how do you do.It is used when you meet someone for the first time.If you really need an answer.Very well thank you or pleased to meet you,Well thank you and you?

You say "Hey friend, "How do you do?" is for the first time meeting someone, and the reply then is "How do you do?" What up with that? You don't remember me?"

They are asking you how you feel. Answer them as best you can.
