There's a product I use - I get it from PetSmart - that works like a charm every time. It's called Simple Solution Stain and Odor Remover. Saturate the soiled spot very well (this works if they throw up or go potty as well - just make sure you clean up any solids and blot the area well with paper towels - DO NOT RUB!!)
Once you've saturated the area very well, let it soak up - as it evaporates, not only will it eliminate the smell, but it will help remove the stain also. If it still smells after a day or two, I'd sprinkle Glade Carpet Deoderizer (the Pet Odor Neutralizer one) and then vacuum. Make sure you do the sprinkling of the powder and vacuuming of the carpet ONLY when the Simple Solution step is COMPLETELY dry. Otherwise not only will you make a further mess, but you may risk electrocuting yourself.

Mix a small amount of white vinegar and add about tsp of baking soda (fizzing will take place which is normal), using a cloth, saturate and wring out...dab the area til saturated right down to the underlay area. (this should help lift some of the urine as well if there is any still there), leave it sit for about 10 minutes, dab area again (with pressure) using a clean cloth soaked in plain water. Then use a good carpet cleaner and go over the area again until stain is no longer evident.
Once dry, spray with febreeze....remember you need to do the first steps thoroughly in order to get right down to the undercarpet area...this is where the smell will originate from the longest.
Baking soda and vinegar will neutralize the smell and also help remove some of the staining.

I saw a commercial about pet zero which is supposed to take even skunk oder down to zero there's an email address www.petzero.com. I don't have pet stains in my house but I wonder if it works. Just trying to help.

Don't' know the answer to this because when I have had puppies you teach them to go outside to do there business, but you should have paper but down where there kennel is or basket, this is part of the training, if the puppy is kept indoors.

First try orange eliminator to remove the spot. It does have a nice orange smell to it. If that doesn't work, there is Febreeze
