A ruptured appendix spills bacteria-laden intestinal contents into the abdominal cavity, causing peritonitis (see Emergencies: Peritonitis), which may result in a life-threatening infection. A rupture also may cause an abscess (a pus-filled pocket of infection) to form. In a woman, the ovaries and fallopian tubes may become infected, and the resulting blockage of the fallopian tubes may cause infertility. A ruptured appendix also may allow bacteria to infect the bloodstream—a life-threatening condition called sepsis (see Bacteremia, Sepsis, and Septic Shock: Bacteremia and Sepsis).

The standard treatment for appendicitis is surgical removal of the appendix. In many cases the surgery is straightforward, and you recover quickly. But if your appendix has ruptured, the surgery may be more complicated and you'll take longer to heal. If the appendix ruptures, the infected contents flow into the abdominal cavity, causing a much more serious medical emergency - peritonitis. This is the inflammation of the membranes lining the abdominal wall and organs. Without prompt treatment, peritonitis can be life threatening.

A ruptured appendix that's not promptly treated can lead to serious complications such as infection. In rare instances appendicitis that results in a ruptured appendix can be fatal.

The most serious complication of a ruptured appendix is peritonitis, in which there is an infection outside the intestine and within the abdominal cavity. If not treated properly with antibiotics it could be fatal.

If it actually ruptures, you have a very good chance of it being fatal.
Even if they can control the resulting infection & internal damage & scarring, you'll never be the same.
If in doubt, see a Doctor, or get to an ER.

the poison from it rupturing can kill you if you don't go to a hospital immediately.
