My origin would be from digital logic, specifically what pins on a chip are the "inputs" and what are the "outputs". Unfortunately, I'm guessing this phrase far pre-dates Integrated Circuits.

i can think of the ins and outs being like going in and out of a place enough times to know all about it, maybe?
