EVERYWHERE!!! Staph infections are extremely common, in fact about a third of the world's population are infected with it, even if a lot do not display any problems or symptoms.
But yes...hospitals and labs are usually a good place to find it because of the high concentration of patients affected by it. A staphylococcus aureus infection can range from small skin infections to severe systemic infections that can even be antibiotic resistant...these are the really dangerous kind.
A particular strain of staph that is antibiotic resistant is the Methicillin Resistant Staphylococcus Aureus (MRSA). This type of staph infection is definitely one you want to avoid...it is highly contagious. Hospitals treating patients with this particular type of staph infection put the patient in contact isolation, meaning that everyone that comes in close contact with the patient is required to wear a protective gown, mask and gloves. The treatment for this type of infection can be tedious and tricky as it completely resists mainstream treatment with the usual antibiotics (all penicillins) and some have even evolved to resist vancomycin and teicoplanin treatments (the usual meds used to treat MRSA).
Anyhow...incidence of MRSA is now considerd to be a bit of an epidemic and new antibiotics are being developed to keep up with the resistance that the bacteria are gaining.
As far as prevention...alcohol based sanitizers and hand rubs have been found to be effective...even simple hand washing can increase your protection exponentially. Good times!

Many people have Staph Aureus bacteria on them, especially if they have worked in a hospital. It's not usually a problem unless it enters the body in some way, such as via a wound, but then it can cause infection.

Healthcare professionals should always wash their hands between patient contacts and after touching their noses or hair, as there may be more Staph A bacteria at these sites.

This is more commonly called "staph" and a staph infection is most likely contracted from a hospital but its found on the skin. Each year some 500,000 patients in American hospitals contract a staphylococcal infection.

Staphylococcus aureus (commonly called staph infection) is a spherical bacterium, frequently living on the skin or in the nose of a healthy person, that can cause illnesses ranging from minor skin infections (such as pimples, boils, and cellulitis) and abscesses, to life-threatening diseases such as pneumonia, meningitis, endocarditis, Toxic shock syndrome (TSS), and septicemia.

It is a type of bacterium commonly found on the skin and/or in the noses of healthy people. Although it is usually harmless at these sites, it may occasionally get into the body (eg through breaks in the skin such as abrasions, cuts, wounds, surgical incisions or indwelling catheters) and cause infections. These infections may be mild (eg pimples or boils) or serious (eg infection of the bloodstream, bones or joints).

Foods that are frequently incriminated in staphylococcal food poisoning include meat and meat products; poultry and egg products; salads such as egg, tuna, chicken, potato, and macaroni; bakery products such as cream-filled pastries, cream pies, and chocolate eclairs; sandwich fillings; and milk and dairy products. Foods that require considerable handling during preparation and that are kept at slightly elevated temperatures after preparation are frequently involved in staphylococcal food poisoning.

Im not sure if you are asking where one would contract MRSA or where it evolved from, but MRSA is a nosocomial infection, meaning it is usually contracted in hospitals where other patients with MRSA share space with people who have suppressed immune systems.
