Your brother will be Dr. Lewis

Ways to get Dr in front of your name are by getting an MD, PhD, DVM (Doctorate of Veterinary Medicine, DDS (Dentistry), and similar degrees. Masters degrees holders don't get the Dr. in front of their name.

If you attain a academic degree of the highest traditional level either a PhD- a research doctorate following the aquisition of a masters or an MD- a medical doctorate following some pre-medical studies you get the Dr. prefix. Most people with doctorate degrees have some preference on what they are called, and I see no season why your brother could not ask people to call him Dr, but most people with research doctoral degrees that I have met don't care much for the title in their name.
