Kittichai in Soho at 60 Thompson Street is wonderful! Soups are outstanding.
I've been eating at Pongsri in Chinatown for years - it's still good
(Baxter and Bayard Streets)
I like Holy Basil on First Avenue in the East Village.
I understand there is a Thai restaurant in Jackson Heights and one in Woodside, Queens that puts them all to shame, but I haven't tried them yet.
My favorite pad thai is at Siam Orchid in Sheepshead Bay, Brooklyn.

Yum Yum Thai on 9th Avenue (45th Street) is awesome!
I also really like Viang Ping - it is not fancy, but the food is great
(1393 2nd Ave Between 72nd & 73rd St)
If you want fancy, high end Thai, try Vong, the Jean Georges restaurant (200 E 54th St At 3rd Ave )
Tiny Thai on 9th Avenue has the best Thai iced tea, and really good food too!
Check out menupages.com for lots of great Thai places, all over the city.
