When was the last time you went to the dentist? It is possible you have gum disease and did not know about it. Because you were a smoker (this is from experience) your gum line has been receding (called bone loss) and you have bacteria between your teeth and gums that has been collecting for sometime. If you brush your teeth or floss and have noticed puffy gums or blood seeping you must go see a specialist. They will measure your gum line and if needed perform the necessary clean up.

Two most common problem causing bad breath aside from smoking and the food you eat, are dental problems and infections of the upper respiratory tract (like sinusitis). See you doctor and dentist and if they can't help you an specialist like an otolaryngologist (ENT) might be able to help.
