If its yours call dmv.Its probably an unpaid ticket or tax of some kind.

go to your state licensing board web site or your state American Medical Association web site. Either place should have a list of revoked licenses

Many states have websites for their board of health where you can do a license lookup on a person and find out details regarding their licenses. I would start there.
