Sometimes you'll be able to judge the seriousness of the situation; if you're on the ground floor, you'll likely be able to leave your room no matter what (there's always the windows if the fire's in the hall), so you can get dressed. Sometimes you can't; if you're on the top floor, even if you don't see or smell anything, a fire on a floor below you may cut off your escape route while you're getting dressed. This question is in the ettiquette category but I'd really worry about saving my skin and not about ettiquette!
In summer, or even in winter if you *do* see/smell smoke, especially if you're on a high floor, get the h*ll outa there even if you're fully naked -surely someone will arrange for blankets soon. If you think you have the time, either quickly put on the first warm thing in sight (coat, sweater, who cares) or even just grab some clothes and carry them so you can put them on once you're safe. "He was nicely dressed" isn't the thing you want them to put on your tombstone, and fires in highrise buildings can be very serious.

It depends on the size, age, layout of the building. If you're in any modern building, you should have plenty of time. Smell for smoke, feel the door handle for heat, check the hallway for fire. If all that is clear, then put some clothes on that are appropriate for the outdoor weather, grab your wallet and keys, and head out the door. If the building is over 50 years old, I might be a little more cautious in getting out in time, but even then you probably have more than enough time to put on a minimum of clothes to protect from the elements outdoors. Additionally, if there is a panic amongst other inhabitants, you'll be glad you had shoes on.
But that does remind me of a time in college when the fire alarm was broken and went off several times at all hours in the span of two weeks. Eventually, guys started deliberately shedding their clothes, stepping in the shower to get wet (the excuse not to get arrested) and then going outside naked in protest. The school got the hint.

if the alarm goes off and staff is assisting with an evacuation, you should leave immediately. there were two hotel fires two weekends in a row in Cincinnati recently. these can be serious! If it's easy, grab a robe, your shoes, whatever is on your way out the door, but take it seriously!

If you sleep buff, you should always have slippers and a robe handy just in case. Lay them by your bed so you won't scare the survivors to death.
