Assuming that by "by mistake" you mean that they invaded because of incorrect information, yes, tons of times. World War I leaps to mind; so does most of classical history, a good couple crusades and some of the more embarrassing moments in the Age of Colonialism. Such things are common because countries try to keep each other in the dark as to their intentions. Military types refer to this and related difficulties as "the fog of war."

About Iraq, we've been there since Desert Storm. We had a truce/cease-fire that Saddam regularly violated, including shooting at the Air Force planes in the UN authorized no-fly zones. We could've acted on those violations of the cease-fire to retaliate at anytime in the decade since the end of Desert Storm. WMDs was an overhyped late excuse to be the final straw, but we were already there with the Air Force and didn't need any excuse to roll the tanks in...

Hindsight is always 20/20. In my personal view, and being a Vietnam vet, any war is a mistake. We need better intelligence, less partisan politicians and more diplomacy. In the end, war is an attempt to change a way of life to one that suits the invader.

About the only time I can recall this happening was about 10 years ago when a convoy of Canadian soldiers accidentally crossed the border. The local police spotted the convoy then (thinking they were supposed to be there) began blocking traffic at intersections so the Canadians would not have to stop.
Once the Canadian troops noticed the American police waving them through the intersections they re-checked their maps and turned around.
Apparently nobody on our side of the border took any notice of this so when the Canadian government called to apologize we had no idea what they were talking about.

So, which country did we allegedly invade "by mistake"? If you believe we invaded Iraq "by mistake", then you haven't been paying attention. There were a myriad of valid reasons that supported our actions, and nobody has proven to me there was any "mistake".

After Stalingrad Hitler thougth it was a mistake by invading Russia. After Iwo Jima I think the Japs believed they made a mistake at Pearl Harbor.
