I've had fluid (and blood) drain from my joints more times then I can count. If it feels like there is fluid (aka, very swollen, tender, and you can physically move the fluid around), then chances are, you have fluid (water on the knee). If it's signifigantly swollen and painful, but you can't move fluid around, then you still might have fluid backup - it will be UNDER the knee cap.
The former hurts a bit to get removed - they stick a shallow needle anywhere from just where the fluid is built up to all 4 sides of your knee and they draw it out. Honestly, it hurts like hell to get the later done. They stick a massive needle through your a space between your knee bones and drain it. Either way, It will continue to be swollen for the next day (or, depending on how you heal and how deep the fluid was, it could take up to 4 days).
The next day, however, will be SO much better. Running, jumping, everything is easier.
Either way, having the fluid removed is the only way to get rid of the pain. Sometimes, your body will asorb it naturally...but only if there is a small amount.
Ice it and keep it evaluated untill you can see a doc.
And yes, you need to see a doc. Your family doctor can normally do it.

When I had knee pain, I looked on that site and then went to the doctors. I had to have physiotherapy to sort it out.
