I just made some bread 2 weeks ago with some I had in my fridge over a year, and it turned out ok.

Heat 1/2 cup of water to 100 degrees, stir in 1 tablespoon white sugar. When sugar is completely dissolved, stir in packet of yeast. Stir or whisk the yeast so that it dissolves. After 5-10 minutes there should be a nice foam. This is how to determine if the yeast is still active. If you get the foam, you can now add this to your bread. If there is no foam, then the yeast organisms are dead and your packets are no good.

1 packet of yeast = 2 1/2 teaspoons

I wouldn't waste my time or ingredients if I weren't positive... but you can check it.

Sprinkle it into a cup of warm (110-115ºF) water, and add 1 tsp. sugar. Stirl well and let sit 15 min. When 15 min is up, look at it. If it is foamy and bubly, or milky/cloudy in appearance thru-out, it's fine. If the yeast has settled to the bottom w/ no bubbles, it's dead. If it's active, just add this to your bread dough and proceed as usual.

There should be an expiration date on the jar, check that and if it's expired I wouldn't use it. I had a bit of a disaster using old yeast in my bread machine - came out very weird!

No. Even refrigerated, the yeast will die or at least be diminished significantly after the year. I wouldn't use it.
