i lost my pass-port, what do i need to do in order to get another one? do i need to report it?

For a US passport:

If you lose your US passport and you're in the US (whether or not you think it was stolen) you must report the loss immediately by completing the Statement Regarding Lost or Stolen Passport (Form DS-64) and submitting it to the Dept. of State. You can then apply to have your lost passport replaced, by appearing in person and with the appropriate application form (Form DS-11) at a Passport Agency.

If you lose your passport when traveling outside the country, you must report the loss to the local police and nearest US embassy or consulate. You'll have to prove your identity and fill out an affidavit. The US Embassy will be able to provide a temporary passport that will get you home, and you will then have to get a new one here (US Embassies, since 9/11, no longer provide new passports). If you have a photocopy of the passport that was lost/stolen, more better.

If you're a US citizen, you should report a lost passport immediately. Here is the US State Department website for lost or stolen passports: http://travel.state.gov/passport/lost/lost_849.html If you are a citizen of another country, contact your government or embassy.

you need to go to the police and get a number so u can pass it on to the passport office as proof u have reported it lost

If you are in your home issuing country, I recommend that you report it first to the Police, then go to the passport office or download the appropriate forms and fill it in again, citing lost, to replace it.

If you are abroad, report it to your hotel staff, who will help you report it to the police, then contact your embassy or consulate and help you get home!

I think the nearest British Embassy in the country you are in would be the best place to go except if you lose it in the UK. In the UK just reapply at the passport office. I think they have a website which might be able to help here.

You must report it to the Passport issuing department straight away and also report it to the police.

I would ring the passport office to advise them and get a replacement [passport form from your postoffice and fill it in. I would speak to them though as you may need to get photos signed again. At least it is a chance to update your passport photo!!!

Yes, report it immediately. They will send you the appropriate paperwork and you have to go through the whole process of applying for a new one. (photos etc)

Takes time but you will get a replacement.

Go to your country's Embassy in person, don't apply on teh net or by post. You'll be given a temp document while they get the original from your home country.

You got to report it to the local authorities (read "po-po's") ASAP . More so if you are in a foreign country (and then to your country's embassy). Judging by the lack of urgency in your question, looks like you lost it in your country. In any case you have to inform the po-po's if you are to receive a replacement.
