It means
"How have you been?"
your answer depends on your moods at that particular moment
if you are OK: Muy bien, Gracias. y usted
If you are not fine: No, Muy Bien pero Gracias. y usted
Add the following when you Finnish answering: Que tenga buen dia
which it means the same as thanks, bye- bye, take care ,or have a nice day

Basically means What's up (how have you been). The answer will depend on your moods. The common answer is "Muy Bien, y Usted?"

Bien=good
