Go to your state's DMV office. Bring all the information you have, such as tag numbers and VIN. You also may need the exact owner information, which, if you are the owner, is on your drivers license. Bring proof of insurance, some states will require it.

Depending on the state, it may cost $5-20 for a duplicate registration card, or it may be free.

In my province in Canada it is free if you change addresses or something like that, which is the only reason I have ever done it. I suspect it is free, or of little cost, in most places.
